Once you’ve browsed our provider bios, please contact us to determine clinician availability and next steps. You may contact us via our Contact form, or via email at hello@sustainablewellnessny.com. A member of our intake team will work with you to get the scheduling process started. If using insurance, our Intake Coordinator works closely with our Billing Manager to help verify your benefits.
*Please note that while we actively keep our website as updated as possible, we cannot guarantee availability for any provider or specific time of day. We also cannot guarantee insurance coverage for any service, even if we contract with your insurance provider.

After scheduling your initial intake appointment, our intake coordinator will send you a confirmation email with the link to fill out our new client paperwork. The forms often take about 15-20 minutes to complete, as they include questions that help your therapist get to know you. It's our policy that all new clients complete paperwork at least 24 hours before your first appointment. Sessions cannot start until these intake forms are completed.

If you are meeting with your provider in person, you can visit our Offices page to review details about the office location and find detailed instructions to the office. Teletherapy clients will receive a link from our secure video platform prior to the start of each session.

Our Intake Coordinator is your point person until you complete your intake session. Once you begin services, you will work directly with your therapist regarding treatment planning and ongoing scheduling. At that time, you can always schedule / reschedule sessions through our Client Portal or reception service by calling 585-206-2631.
Rates & Insurance
It’s important to understand the costs involved with therapy, what your insurance covers, and what your financial responsibilities may be. Insurance coverage for mental/behavioral health can depend on a variety of factors. Most plans cover the cost of chiropractic care for acute (short-term) conditions. Be sure to talk with your insurance company to determine your benefits prior to starting services.

Fees

Any out-of-pocket expenses can be paid via cash, check, credit card, or HSA/FSA card and are due at the time of service. Even when using insurance, there may be out-of-pocket fees, such as copays, co-insurance, or deductibles. Missed appointment fees are never covered by insurance. Clients choosing to forgo insurance or have an out-of-network provider are subject to services at the self-pay rates listed below. Please note that not all of our services are covered by insurance, and therefore are self-pay only. The No Surprises Act helps to ensure you will never be surprised by your bill or cost of payment.

If you have another out-of-network insurance provider, you may request a Superbill to submit to your insurer for reimbursement. We cannot guarantee reimbursement from any insurance company, even those we contract with. We strongly encourage you to check with your insurance provider to confirm any outpatient mental health coverage prior to starting services with us.

We do not accept Medicaid, Medicare or any Medicaid-funded products such as Blue Choice Option (BCO) or Child Health Plus.
